Appointment Process
You will be glad to know that we are currently accepting new patients. You can call our office to schedule a new appointment. We maintain a clear appointment system, so if you have called for an appointment, the time slot will be reserved for you.

List of Things Required for Your First Appointment

- Any medical records pertaining to your visit
Please arrive 15 minutes before your scheduled appointment time to fill out necessary paperwork.
Insurance Requirements
You must carry your insurance information with you for your first appointment. Our experienced staff will complete the necessary paperwork required for claims and work on getting you the maximum amount of benefits based on your plan.
Please note, payment is expected at the time services are rendered, unless other arrangements have been made. This includes co-payments or deductibles (amount which is not covered by insurance).
